I want to put a new shaft in my Maltby KE4 ST-2A Driver and do not have an extra shaft adaptor.  Any out there?

Also, does anyone have a proven or best way to remove a shaft from an existing adaptor.  I tried the usual heat and pull method and it was just a mess.  I then cut the shaft and drilled it out.  Another mess. Trying to pull the shaft from the adaptor after heating it with torch resulted in the adaptor getting a bit mangled and all the paint and marking on the upper part of its tube being scraped off.  Of course the companion mini-ferrule was also toast.   So I have the adaptor but externally it looks like crap.
